Frequently asked questions
Frequently asked questions about information requests from third parties
Do you have questions about information requests from third parties? We are happy to answer the most frequently asked questions. If your question is not listed, you can of course always contact us.
Is answering information requests from third parties one of the GP practice's tasks?
Answering information requests from third parties falls outside our primary task within GP care. Our focus is on direct care for our patients.
Why can't the practice respond immediately to requests from third parties?
Given our limited time and resources alongside our regular work, we cannot respond to such requests immediately. After all, direct patient care always comes first. We ask for your understanding and patience, and we aim to respond as quickly as we can.
How does the practice ensure information security when providing data to third parties?
We maintain strict information security standards to comply with privacy legislation and ensure that information provided to third parties complies with the applicable regulations.
Why is the patient's consent needed to provide information to third parties?
In accordance with laws and regulations, we only provide information with the written consent of the patient.
How are costs calculated for information requests from third parties?
The costs of requesting information are charged to the requesting party in accordance with the guidelines of the KNMG (Royal Dutch Medical Association).
What requirements must a request for information meet?
A clear, written question is required, as is the (written) consent of the patient for passing on this information. The invoice for preparing this information is enclosed with the information provided. The invoice must be paid within the stated period.
Why is it advisable to be cautious about sharing the complete medical record with third parties?
If the entire medical record is provided, all privacy-sensitive medical information can be viewed by third parties who do not have a professional duty of confidentiality (as a doctor does). Moreover, not all medical information is needed to deal with the matter for which the medical information is being requested.
Providing irrelevant information can also have unintended consequences for the patient, such as harm to their current case or in the future.
How does the procedure for transferring records to a new GP work?
Digital request from the new GP
The record can only be sent securely and digitally to the new GP if the new GP - after registering you - submits a digital request for it. This is essential for correctly linking the systems and for the secure transfer of records.
Timing of sending
If the new GP has not yet submitted the request to send the record, the record cannot be sent yet. We wait for the formal request from the new GP.
ZMFT request
If your new GP does not yet have your record, ask the new GP whether they have submitted a "ZMFT request" to us / your previous GP. This is a digital request for the record. The record can only be sent digitally via this “ZMFT request”. Only the new GP can make this request.
Why the request is needed
The "ZMFT request" is necessary for sending and transferring the record securely. This request can only be created by the new GP themselves.
Questions or problems
If anything about the process is unclear or there are problems with the transfer of the medical record, ask the new GP to contact us by phone. That way, we can work out together what is causing the problem and how we can solve it, to ensure a smooth transfer of your record.
