Practical information
Your privacy comes first
We handle your personal data with care. Here you can read which data we process, why we do so and what rights you have.
General
The General Data Protection Regulation (GDPR, in Dutch: AVG) protects your privacy and personal data. Under this law, an organisation that processes personal data has certain obligations. The person the data belongs to has certain rights. In addition, specific rules apply to healthcare, including those in the Medical Treatment Contracts Act (WGBO). This privacy statement informs you about your rights and our obligations under the GDPR and the WGBO.
Why we process your data
In our practice we process various personal data about you. This is necessary to give you proper medical treatment and to handle the financial side of that treatment. Sometimes processing is also necessary to prevent serious danger to your health, or to comply with a legal obligation. For example, the mandatory reporting of an infectious disease under the Public Health Act (Wet publieke gezondheid).
Our obligations
Under the GDPR, GP practice Hohmann & de Vet is responsible for the processing of personal data in the practice. This means:
- We only collect your data for specific purposes: providing care, efficient management and policy, and supporting scientific research, education and public information.
- In principle, we do not use your data for other purposes.
- We let you know that we process your personal data, through your healthcare provider, a leaflet or our website.
- All our staff have committed themselves to treating your data confidentially.
- We protect your data well against unauthorised access.
- We do not keep your data longer than necessary for good care.
By law, we in principle keep medical data for 20 years after the last treatment, unless keeping it longer is necessary, for example for your health or that of your children. The practitioner treating you decides this.
Your rights
- You have the right to know whether we process your personal data, and which data.
- You have the right to access your data and to receive a copy, insofar as this does not harm someone else’s privacy.
- You can have your data corrected, supplemented or deleted if necessary.
- You can ask for (partial) destruction of your medical data. This is only possible if keeping the data is not of significant importance to someone else and the law does not require it to be kept.
- You can have a statement of your own (of a medical nature) added to your file.
- In certain cases, you can object to the processing of your data.
Would you like to exercise your rights? Let us know in person or use the request form at the bottom of this page. A representative, such as someone you have authorised in writing, a guardian (curator) or a mentor, can also do this on your behalf.
Notes on the request form
Please fill in the form as completely as possible; this helps us find your file and protect your privacy. We treat your data in strict confidence. We are not liable for errors in postal delivery. Would you rather collect the file yourself or have an authorised person collect it? Please indicate this on the form.
Fill in the details of the person the file is about. Under the WGBO, you are responsible for yourself from the age of 16: young people aged 16 and over submit a request for access or a copy themselves. If the patient has died, medical data may only be provided if it can be assumed that the deceased would not have objected, or if there are compelling reasons to break medical confidentiality. The healthcare provider decides this.
Sharing data with others
Our staff treat your data confidentially. We need your explicit consent to share your data with others. There are a few exceptions: if the law requires us to do so, or if there is a serious threat to your health or that of someone else. When necessary, we also share data with other healthcare providers involved in your treatment. For example, with the pharmacist who prepares your prescription.
Exchange via the Landelijk Schakelpunt
If you have given your consent, we securely share relevant medical data with the huisartsenpost (out-of-hours GP service) via the Landelijk Schakelpunt (LSP, the national system for exchanging medical data). If you have been to the huisartsenpost in the evening or at the weekend, they send us a message. That way your GP knows what complaints you had there and what was done.
We can also share medication data with your pharmacy and the specialists treating you: the medicines your GP has prescribed and any intolerances, contraindications and allergies. Other prescribers can then take these into account. This is how we contribute to the safe use of medicines. You can read more about this in the questions about the LSP.
Transferring your file
If you choose a new GP, it is important that they know your medical history. That is why your old GP transfers your file to your new GP, as soon as possible and in any case within one month of your request. This is done securely: in person, via a secure digital connection or by registered post. You cannot take the original file with you, but you always have the right to access it and to receive a copy.
Cookies on this website

Questions or complaints
Do you have a question or complaint about how we handle your data, or about who we share data with? Your GP will be happy to discuss this with you.
