Frequently asked questions

How the digital request from a new GP works

Do you have questions about transferring digitally to a new GP? Below we explain how it works. If you still have questions after that, you can always contact us.

How does the procedure for transferring my data to a new GP work?

Digital request from the new GP
Your record can only be sent securely and digitally to the new GP if the new GP – after registering you – submits a digital request for it. This is essential for the systems to be linked correctly and for the records to be transferred securely.

Timing of sending
If the new GP has not yet submitted the request to send the record, the record cannot be sent yet. We wait for the formal request from the new GP.

ZMFT request
If the new GP does not yet have your record, please ask the new GP whether they have submitted a "ZMFT request" to us / your previous GP. This is a digital request for the record. The record can only be sent digitally via this “ZMFT request”. Only the new GP can make this request.

Why the request is needed
The "ZMFT request" is necessary for the secure sending and transfer of the record. This request can only be created by the new GP themselves.

Questions or problems
If anything about the process is unclear or there are problems with the transfer of the medical record, please ask the new GP to contact us by phone. That way we can look together at what is causing the problem and how we can solve it, to ensure a smooth transfer of your record.

Questions or need an appointment?

Call the practice on weekdays between 8:00 and 17:00, or book an appointment yourself via Uw Zorg Online. For urgent matters outside opening hours, call the out-of-hours GP service (huisartsenpost).
