Register with our practice

We are temporarily open for new registrations from residents of Kralingen. Please fill in the form completely; we will then contact you.

Who can register? Residents of the Kralingen district (postcodes 3061, 3062 and part of 3063). Not for residents of De Esch and Nieuw-Kralingen. Due to high demand, we only register patients who do not yet have a GP in Kralingen.

Filling in the form takes about 10 minutes. Fields marked with * are required.

After you register, we will request your medical record digitally from your current GP. Please therefore fill in the details of your current GP practice.

Let your current GP know yourself that you are leaving. Our system requests your record automatically with a secure message, but without a personal message from you, your current GP will usually not deregister you or send your record.

Would you also like to register family members? Please fill in a separate form for each family member.

Through the Landelijk Schakelpunt (LSP, the Dutch national exchange point for medical records), other healthcare providers, such as the huisartsenpost (out-of-hours GP service) and your pharmacy, can view important medical information when necessary. Read more about the LSP.

A photo or scan of up to 2 MB (jpg, png or pdf). Tip: with the KopieID app from the Dutch government (Rijksoverheid), you can make a secure copy.

What you will need

  • A copy of your proof of identity
  • Your health insurer and policy number
  • The details of your current GP
  • An overview of your medicines and conditions

Good to know

Please fill in a separate form for each family member. After you register, we will request your medical record from your current GP. Please also let your current GP know yourself that you are leaving.
