You are in direct contact with the doctor’s assistants and doctors of our practice: the trusted team you already know.
Chat with the practice
Send us a message whenever it suits you. No waiting on the phone.
Quick reply
We usually reply within one working day. When it is busy, it may take a little longer.
Send photos and files
For example, send a photo of a rash or wound, so we can quickly assess what is needed.
Video consultation
A consultation with video through the app, without separate software.
Fill in questionnaires
Prepare for your consultation by describing your symptoms in advance. The GP is then informed straight away.
Appointment at the practice
Let us know in the chat that you would like to come in. The doctor’s assistant will then schedule a suitable time with you.
Getting started
Ready to use in four steps
Registering takes less than five minutes. You need a valid email address and your BSN (citizen service number).
- Download the app from the App Store or Google Play. The app is free.
- Create an account with your name, email address and date of birth. You will receive a confirmation link by email.
- Link our practice: search for “Hohmann” or postcode 3061 PN and send a link request.
- Start chatting as soon as we have approved your request.
Download the Cory.Care app
Free for iPhone and Android. Your data is processed securely on Dutch servers. The app is in addition to regular care: you can always still call us or visit the practice.

Questions about Cory.Care
Is Cory.Care secure?
Yes. Cory.Care complies with Dutch and European privacy legislation (GDPR). All data is stored on secure Dutch servers with NEN 7510 and ISO 27001 certification.
What does Cory.Care cost?
Nothing. Using Cory.Care is free for you as a patient.
Do I need to register again?
No. Cory.Care is in addition to the care at our practice. You simply remain registered with us.
How quickly will I get a reply?
We aim to reply to your message within one working day. When it is busy, it may take a little longer.
Can I use the app for urgent matters?
No. If life is in danger, call 112. For other urgent matters, call the practice on weekdays and choose 1 for the urgent line. In the evening, at night and at weekends, call the out-of-hours GP service (huisartsenpost).
Questions or need an appointment?
Call the practice on weekdays between 8:00 and 17:00, or book an appointment yourself via Uw Zorg Online. For urgent matters outside opening hours, call the out-of-hours GP service (huisartsenpost).
