Frequently asked questions about your medical record

Do you have questions about your medical record? We are happy to answer the most frequently asked questions. If your question is not listed, you can of course always contact us.

Can I have information removed from my medical record?

Yes, you have the right to have certain information removed from your medical record. To make this process as efficient as possible, we offer the option of requesting a copy of your record. You can indicate which specific information you would like removed by highlighting the relevant passages on the copy with a marker pen. Once you have handed this in, we will remove the highlighted passages from your record. It is important to mention that, at the place where information was removed at your request, we will note that passages were removed at your request, which means the record becomes incomplete as a result. This note is made for the sake of transparency and in accordance with the right to access and correct your medical data. You can submit a request to collect the copy of your record and state your specific wishes regarding removal. If you have any further questions about this process, we are available to clarify and support you.

As a parent, can I have information destroyed from my underage child's GP record?

As a parent, you have the right to have information destroyed from your underage child's GP record. However, the process for this depends on parental authority. If both parents have joint authority, consent from both parents is required for the destruction of (part of) the record of a child under 12. Between the ages of 12 and 16, the child's consent is also needed. It is important to know that destroying a record is a drastic and irreversible step. That is why written consent is requested from all parents involved. In situations where a parent's request is clearly contrary to the child's interests, the doctor retains the right to refuse the request on the basis of good care practice. If the parents with authority disagree, the doctor cannot grant the request. The doctor can refuse a request if it is clearly contrary to the child's interests and/or the parent is pursuing their own interests.

How long does it take for the changes to my record to be processed after I have requested the destruction of (parts of) my record?

We aim to send an acknowledgement of receipt as soon as possible after we have received your request to destroy (parts of) your record. In principle, you can expect a response within one month. If the request is very complex, we reserve the right to extend this period by another month. The time needed to respond to your request depends on its complexity and on the time available alongside our regular care tasks.

This is in line with the guidelines of the KNMG (Royal Dutch Medical Association), the organisation that sets rules for handling such requests in healthcare. If you have not received a response within the expected time, please feel free to contact us for more information.
