Frequently asked questions about COPD check-ups and medication policy
Do you have questions about COPD check-ups and medication policy? We are happy to answer the most frequently asked questions. If your question is not listed, you can of course always contact us.
Why do I have to fill in a questionnaire every year if I have COPD?
Your health comes first for us, and we aim to keep your COPD well under control. The GP is responsible for the medication policy, and regular evaluation is essential to make sure the prescribed medication matches your needs as closely as possible. That is why your GP invites you every year to fill in the COPD-meter, a simple questionnaire that is available online.
How do I fill in the COPD-meter?
Once a year you will receive a secure e-mail from VIP-Calculus with a link to the COPD-meter. With just 16 questions, you can make a valuable contribution to your COPD check-up in a few minutes. The GP will assess your completed questionnaire.
What if I cannot or do not want to fill in the COPD-meter?
If there are reasons why you cannot or do not want to fill in the COPD-meter, we would like to hear about them. Send us a message by e-mail to poh@hohmanndevet.nl or via the patient portal of Uw Zorg Online. Your feedback is valuable for an appropriate approach to your care.
What happens if there are changes in my COPD control?
If the COPD-meter assessment or other COPD check-ups give reason to do so, we will contact you. Together with the GP or practice nurse, we will look at how your COPD treatment can be better adjusted. If your COPD is well controlled, no further action is needed and you will receive another invitation for the COPD-meter next year.
Can I contact you if I have questions?
Certainly! If you have questions, you can always contact the GP. Your health is important, and we are ready to answer all your questions and support you.
