Frequently asked questions about chronic care - integrated care for diabetes and CVRM
Do you have questions about chronic care, integrated diabetes care and CVRM (cardiovascular risk management)? We are happy to answer the most frequently asked questions. If your question is not listed, you can of course always contact us.
How is chronic care for (a risk of) cardiovascular disease organised?
If we know that you have (a risk of) cardiovascular disease, you are advised to come for a check-up at least once a year with the practice nurse Jettie Laarman at our practice. How do check-ups with the practice nurse (POH-S) work nowadays?
1. You are registered with Star-SHL (the laboratory) for an automatic reminder service. This is useful, because it means neither we nor you will forget that it is time for a check-up again.
2. When it is time for the annual check-up, Star SHL will invite you by letter to have a blood test. You are usually invited for the annual check-up in the month of your birthday.
3. One week after the blood test, make an appointment for a double consultation with Jettie Laarman
If you prefer to make an appointment online, you can also do so via the UwZorgOnline app (our patient app). You then make 2 consecutive appointments (40 minutes). (see also Patient portal - GP practice Hohmann & De Vet in Rotterdam (uwzorgonline.nl))
During the check-up, we discuss your blood test results, take measurements (blood pressure and weight) and talk with you about your medication and your lifestyle.
How is chronic care for diabetes organised?
If we know that you have type 2 diabetes mellitus, you come for a check-up between 1 and 4 times a year with the practice nurse Jettie Laarman at our practice. How do check-ups with the practice nurse (POH-S) work nowadays?
1. You are registered with Star-SHL (the laboratory) for an automatic reminder service. This is useful, because it means neither we nor you will forget that it is time for a check-up again.
2. When it is time for the standard check-up, Star SHL will invite you by letter to have a blood test.
3. One week after the blood test, make an appointment for a consultation with Jettie Laarman. For the annual check-up you make a double appointment, and for an interim check-up a single appointment.
If you prefer to make an appointment online, you can also do so via the UwZorgOnline app (our patient app). (see also Patient portal - GP practice Hohmann & De Vet in Rotterdam (uwzorgonline.nl))
During the check-up, we discuss your blood test results, take measurements (blood pressure and weight) and talk with you about your medication and your lifestyle.
Can a check-up also take place online instead of coming to the practice?
Yes, that is possible. After your blood has been taken for the check-up, the follow-up can now also take place online. This offers convenience and flexibility, so you can do the check-up at a time that suits you best. If you prefer online check-ups, please let our practice nurse Jettie Laarman know via POH@hohmanndevet.nl. Make sure you have a good blood pressure monitor and that you have given us your current email address. This way we stay well informed and together we can make sure you get the right personalised care. Thank you for your cooperation!
I want to buy a blood pressure monitor for home use. Which blood pressure monitors are reliable?
You can buy a reliable blood pressure monitor from as little as 25-30 euros. Visit the website of the Hartstichting (Dutch Heart Foundation) for information about reliable blood pressure monitors.
