Frequently asked questions about asthma check-ups and medication policy
Do you have questions about asthma check-ups and our medication policy? We are happy to answer the most frequently asked questions. If your question is not listed, you can of course always contact us.
Why do I have to fill in a questionnaire every year if I have asthma?
We aim to keep your asthma well under control. Your GP is responsible for the medication policy, and regular evaluation is essential to make sure that the prescribed medication suits your needs as well as possible. That is why your GP invites you every year to fill in the Asthma Control Questionnaire (ACQ), a simple questionnaire that is available online.
How do I fill in the ACQ questionnaire?
Once a year you will receive a secure email from VIP-Calculus with a link to the ACQ questionnaire. With just 6 questions, you can make a valuable contribution to monitoring your asthma in a few minutes. We will assess your completed questionnaire. If the questionnaire shows that your asthma could be better controlled, we will contact you
What should I do if I cannot or do not want to fill in the ACQ?
If there are reasons why you cannot or do not want to fill in the ACQ, we would like to hear about them. Send us an email at poh@hohmanndevet.nl or a message via the patient portal of Uw Zorg Online. Your feedback helps us provide the right care for you.
What happens if there are changes in how well my asthma is controlled?
If the ACQ assessment or other asthma check-ups give reason to do so, we will contact you. Together with the GP or practice nurse, we will look at how your asthma can be brought under better control. If your asthma is well controlled, no further action is needed and you will receive another invitation for the ACQ next year.
Can I contact you if I have questions?
Certainly! If you have any questions, you can always contact the GP practice. Your health is important, and we are here to answer all your questions and support you.
